The Department can provide long term care in a community setting through several Medicaid home and community based waiver programs. These programs provide an alternative to long term care services by allowing Medicaid payment for services that are traditionally provided in an institutional setting. The Home and Community Based Waiver Programs are administered by the Department’s Social Services Division/ Adult and Community Services Branch. The waiver programs include the:
- Nursing Home Without Walls Program (NHWW) which provides services in an individual’s home.
- Residential Alternative Community Care Program (RACC) provides long term care services for adults in need of a residence. Services may be provided in an Assisted Living Facility or a RACC family home.
- Developmentally Disabilities/Mental Retardation Program (DD/MR) provides services to individuals with developmental disabilities/mental retardation that would enable them to live as independently as possible in the least restrictive environment.
- Medically Fragile Community Care Program (MFCC) provides services for children with complex medical problems. Services are provided in the family home or in a licensed child foster home.
- HIV/AIDS Community Care Program (HCC) provides services for individuals who are diagnosed with HIV/AIDS infection.
- Program of All Inclusive Care for the
Elderly (PACE) is a managed care program for individuals over
age 55 who live in Maui.
ELIGIBILITY
The Social Services Division will evaluate applicants for the waiver programs for enrollment in the appropriate program. Applicants for the waiver programs must be eligible for Medicaid, require nursing facility level of care, and meet the requirements of the individual waiver programs.
COVERED SERVICES
Recipients in the waiver programs receive all of the coverage afforded to Medicaid recipients. Medicaid will generally not cover long term care services for a recipient who is not residing in a medical institution, except for recipients in one of the home and community based waiver programs. The services provided by the waiver programs may include, but are not limited to:
- Case management
- Attendant care
- Private duty nursing
- Environmental accessibility adaptations
- Day care
- Respite
- Specialized medical equipment and supplies
- Non-medical transportation
